SECTION 1 – AIMS AND OBJECTIVES OF THE DENTAL PRACTICE

1. We will aim to provide high-quality dental care including periodic examinations and treatment, where required. We will promote good oral health to patients attending our practice for care and advice. We will involve other professionals in the care of our patients, where this is in the patient’s best interests, for example, referral for specialist care and advice. We will ensure that all members of our team have the right skills and training to carry out their duties competently and with confidence. We will ensure our team has an awareness of current national guidelines that affect the way we care for our patients.

2. We will aim to provide a comprehensive service to all patients without discrimination. Our treatments will be based on evidence-based treatments, and we will use good quality modern materials and approved techniques.

3. Whilst not always possible, we will aim to see our patients on time and will keep them informed if we are running late. Where appropriate and without breaching confidentiality, we will offer an explanation.

4. We will endeavour to spend sufficient time with patients to ensure that they feel that they have been listened to and any concerns addressed.

5. We will aim to be open and fair with all our patients and welcome open communications which help us to continue our services in a manner approved by the majority and will constantly strive to improve the service we provide.

6. We will ensure that we always have adequate numbers of appropriately trained staff available in order for a prompt and caring environment to be maintained.

7. We will have policies for consent and confidentiality that will be regularly discussed at practice meetings and updated when necessary and at least yearly. We will ensure all team members understand the role they play in obtaining valid consent and in maintaining confidentiality.

8. We will ensure that any adverse or significant events are fully investigated. We will aim to learn from these events and make policy changes where necessary to reduce the likelihood of them being repeated or, if possible, eradicated.

9. We will undertake ongoing patient satisfaction surveys that specifically ask patients whether they are confident they are treated as individuals, listened to and involved in decisions about their treatment. We will also capture ad hoc patient comments and discuss these at practice meetings. We will act on all feedback received and ensure that patients are aware of any changes made as a result of their feedback.

10. We will have a published complaints policy that is regularly reviewed and updated. We view complaints as an opportunity to increase the influence our patients have on how we plan and deliver our services. Our Practice Complaints Policy will be displayed, understood and complied with by all team members. Our team training will ensure that all team members understand the importance of listening to the nature of any complaint and reassuring the patient that it has been understood and will be acted upon within the agreed timeframe.

We will act promptly on all complaints or expressions of dissatisfaction and aim to resolve all complaints in-house to the satisfaction of all concerned. All complaints about service levels will be discussed at practice meetings with the aim of rectifying any problems and avoiding a recurrence. We will keep a log of all complaints and analyse this for trends that show where changes to our services may be needed.

11. We will recruit team members with the appropriate qualifications and skills to fulfil their roles in the practice. All new staff members will receive comprehensive general and role-specific induction training and thereafter regularly updated training appropriate to their needs. We will undertake annual appraisals to meet all staff’s training needs. Our recruitment policy avoids discrimination and new team members
are not permitted to start work until all the appropriate checks have been undertaken.

12. We take safeguarding issues very seriously indeed. The practice will have a comprehensive policy for safeguarding and protecting children and adults who may be at risk of abuse. The team will be fully trained in the practice’s policies, including what action to take in the event that abuse is suspected or confirmed. Safeguarding will feature regularly on practice meeting agendas, and we will rehearse what we would do in the event of concerns about potential abuse or safeguarding issues. Team members will all understand the role they play in being vigilant in relation to safeguarding. The contact details for the local safeguarding boards will be readily available in the practice, and all team members will know how to find these details.

Fusion Dentistry provides private dental services including:

  • Routine dental examinations for existing patients.
  • Preventive dentistry and hygiene services.
  • Restorative dentistry including crowns and bridges.
  • Cosmetic dentistry including veneers and smile makeovers.
  • Dental implant assessment, placement and restoration including full arch implant treatments – All on X.
  • Conscious sedation.

Where appropriate, patients may be referred to external specialists or healthcare providers.

The practice provides services primarily to:

  • Adults aged 18 years and over.

  • Dentally anxious.
  • Requiring complex implant rehabilitation.
  • Seeking cosmetic dental treatment.

Michael has over 10 years’ experience in dental practice management and works closely with the clinical team to ensure the service is safe, effective, well-led and responsive to patient needs.

SECTION 8 – PATIENTS VIEWS

We place a strong emphasis on listening to our patients to ensure the care we provide is of the highest standard and reflects their needs and expectations. Gathering patient views will be a routine and ongoing part of how we monitor and improve the quality of our services.

Arrangements for Seeking Patient Views:

  • Feedback Forms: Printed and digital feedback forms will be available in the waiting area and on our website, allowing patients to share their experiences anonymously or with their details if they choose to do so.
  • Post-Treatment Surveys: Patients will be invited to complete a short satisfaction survey following key treatments or new patient appointments to gather insight into their clinical and customer service experience.
  • Verbal Feedback: Clinical and front-of-house staff will be encouraged to engage with patients about their experience, with informal feedback recorded and reviewed by the management team.
  • Complaints and Suggestions Process: A clearly communicated procedure will be in place to welcome suggestions or handle complaints. Patients will be made aware that their feedback is taken seriously and used to drive improvements.
  • Online Reviews: We will actively monitor online reviews (e.g., Google, social media) and respond professionally to both positive and negative feedback, where appropriate.

Using Patient Feedback to Improve Quality:

All patient feedback will be:

  • Reviewed regularly by the practice management team.
  • Discussed in team meetings to identify areas of good practice and opportunities for improvement.
  • Used to inform staff training, service development, and updates to patient communication and facilities.

By integrating patient feedback into our quality assurance process, we aim to deliver responsive, patient-centred care that evolves with the needs and expectations of those we work with.

SECTION 9 – OPENING HOURS

Monday to Thursday – 9:00 am to 5:00 pm.

Friday – 9.00 am to 2.30 pm.

Earlier appointments may be arranged for complex surgical procedures.

Telephone lines are open from 8:30 am to 5:00 pm Monday to Thursday & 8.30 am to 2.30 pm on Friday.

Implant patients are provided with the treating surgeon’s mobile telephone number for urgent implant post-operative concerns.

Outside normal opening hours:

  • Calls are managed by an external provider (Answer4U).
  • Messages are emailed to the practice team.
  • Emails are monitored regularly by a member of staff.

Patients may be directed to appropriate emergency dental services where necessary.

SECTION 10 – ARRANGEMENTS FOR DEALING WITH COMPLAINTS

Fusion Dentistry operates a formal complaints procedure.

Complaints are managed by: Rachel Gleeson.

The Fusion Dentistry Code of Practice for patients who wish to raise concerns.

In Fusion Dentistry, we place great emphasis on meeting and whenever possible, exceeding our patients’ expectations. We try to ensure that all patients are pleased with their experience of our service and we take any concerns a patient may have very seriously.

If you have a concern regarding any aspect of your care, please let us know. We will do all that we can to resolve your concern to your satisfaction promptly and professionally.

Our aim is to respond to your concerns in a caring and sensitive way.

The person responsible for dealing with any concerns about the service we provide in this practice is Rachel Gleeson.

If you raise an issue that you are concerned about, or unhappy with, on the telephone or at the reception desk, after listening to a description of the problem, the person with whom you raise your concern will try to resolve the issue to your satisfaction immediately.

If the person with whom you raise your concern is unable to resolve the issue for any reason, we will contact Rachel Gleeson immediately. Should Rachel Gleeson be unavailable at the time, we will advise you when they will be available, and arrangements will be made for you to meet or speak with them.

If for any reason Rachel Gleeson is going to be unable to meet you or speak with you at a time that is convenient to you, we will arrange for another suitable member of the team to take responsibility for dealing with your concern or complaint.

The team member with whom you first raised your concern will take brief details from you and pass these to Rachel Gleeson so that they can familiarise themselves with your concern prior to meeting you or speaking with you.

If you choose to write to us to express your concern rather than raising it verbally, your letter or email will be passed on to the relevant person immediately.

We will acknowledge any concerns you raise in writing within two days, and we will also include a copy of this Code of Practice with our response. We will investigate your concern and report back within ten working days of it being received. If we are unable to complete our investigations within ten working days for any reason, we will notify you, giving reasons for the delay and the likely period within which the investigation will be completed.

We will confirm the outcome of the investigation and any decisions made in writing.

We will keep proper and comprehensive records of any concerns or complaints received.

We will do all we can to resolve your issue, concern or complaint to your satisfaction. If, for any reason, you are not satisfied with the outcome or the procedure, we will advise you of other avenues open to you for raising concerns.

The practice is committed to safeguarding patients from abuse or harm.

Privacy Policy for Fusion Dentistry for Patient Data

Fusion Dentistry takes great care to protect the personal data we hold for our patients in line with the requirements of the Data Protection Act 2018, which is the UK’s implementation of the General Data Protection Regulation (UK GDPR).

The purpose of collecting and storing personal data about our patients is to ensure we can:

• Provide appropriate, safe and effective dental care, treatment and advice for all our patients.
• Fulfil any contracts we hold in relation to their care.
• For business administration of their care.

Personal data held for our patients

The personal data we process (processing includes obtaining the information, using it, storing it, securing it, disclosing it, and destroying it) for our patients includes:

  • Name, address, date of birth.
  • Unique identification number.
  • Next of kin.
  • Email address.
  • Phone numbers.
  • GP contact details.
  • Occupation.
  • Medical history.
  • Dental care records.
  • Photographs.
  • Family group.
  • Payment plan details.
  • Financial information.
  • Credit card receipts.
  • Correspondence.
  • Details of any complaints received.

We keep an inventory of personal data we hold on our patients, and this is available for patients on request. A list of personal information held is also included in our Privacy Notice that is given to all patients.

National Opt-out Policy

At Fusion Dentistry, sensitive personal information relating to our patients is only used to provide dental care for the individual. It is never shared for research purposes or any non-clinical need. The National Opt-out Policy introduced in March 2020 is therefore not operated at our practice on this basis.

Should we change our policy to use information for a non-clinical purpose or a research project, we would then introduce the National Opt-out Policy.

Disclosure to third parties

The information we collect and store will not be disclosed to anyone who does not need to see it.

We will share our patients’ personal information with third parties when required by law or to enable us to deliver a service to them or where we have another legitimate reason for doing so. Third parties we may share patients’ personal information with may include:

  • Regulatory authorities such as the General Dental Council & Healthcare Inspectorate Wales.
  • NHS Local Authorities.
  • Dental payment plan administrators.
  • Insurance companies.
  • Loss assessors.
  • Fraud prevention agencies.
  • In the event of a possible sale of the practice at some time in the future.

We may also share personal information where we consider it to be in a patient’s best interest or if we have reason to believe an individual may be at risk of harm or abuse.

Personal privacy rights

Under the UK GDPR and the Data Protection Act 2018, all individuals who have personal information held about them have the following personal privacy rights:

• Right to subject access.
• Right to have inaccuracies deleted.
• Right to have information erased.
• Right to object to direct marketing.
• Right to restrict the processing of their information, including automated decision-making.
• Right to data portability.

Patients who wish to have inaccuracies deleted or to have information erased must speak to the dentist who provided or provides their care.

Legal basis for processing data held about patients

The UK GDPR requires us to state the legal basis upon which we process all personal data for our patients, and it requires us to inform our patients of the legal basis on which we process their personal data. This is clearly stated in our privacy notice that is given to all patients.

The legal bases for recording individual types of data are recorded in our patient personal data inventory. This is available for all patients to see on request.

The legal basis on which we process personal information for our private patients is ‘legal obligation’.

The legal basis on which we process personal information for our payment plan patients is ‘contract’.

The legal basis on which we process personal information for our NHS patients is ‘legal obligation’.

Automated decision making

All individuals who have personal data held about them have a right to object to their personal data being subjected to automated decision making.

Patients will always be asked to give specific, informed, verifiable, opt-in consent for any processes involving automated decision making.

Consent

Fusion Dentistry always obtains specific, unambiguous opt-in consent from all patients to whom we send direct marketing information.

Where patients have not previously consented to or have withdrawn their consent for us to contact or share or process their data, we will assess whether the need for processing is proportionate and necessary in relation to the risk to the individual and/or the public if the information is not shared.

Withdrawal of consent

Patients who have given their opt-in consent have a right to withdraw their consent at any time. Patients are advised of their right to withdraw their consent for anything they wish to withdraw from in our privacy notice.

Retention period

This practice retains dental records and orthodontic study models while the patient is a patient of the practice and, after they cease to be a patient, for at least eleven years, or for children until age 25, whichever is the longer.

Complaints

All individuals who have personal data held about them have a right to complain. All complaints concerning personal data should be made in person or in writing to Rachel Gleeson. All complaints will be dealt with in line with the practice complaints policy and procedures.

If you are unhappy with the resolution of your complaint, you have the right to raise your complaint with the Information Commissioner’s Office (ICO).

Transferring personal data outside the EU

Patients’ personal data is not transferred outside the EU.
